Summary
MetGlizer™-VG 2/0.2 is an oral antidiabetic combination therapy designed to manage Type 2 Diabetes Mellitus. It combines the insulin-sensitizing action of Metformin with the insulin-secretagogue effect of Glimepiride, providing dual glycemic control by reducing hepatic glucose production and enhancing insulin secretion.

Usage
-
To be used in patients with Type 2 Diabetes Mellitus not adequately controlled by diet, exercise, or monotherapy.

Taken orally with meals to reduce gastrointestinal side effects and hypoglycemia risk.
Concerns
-
May cause hypoglycemia, especially in elderly or patients with irregular meal patterns.

Risk of gastrointestinal disturbances due to metformin.
-
Long-term metformin use may reduce Vitamin B12 absorption.
When not to use?
Avoid in patients with:

Type 1 Diabetes Mellitus or Diabetic Ketoacidosis
-
Severe renal impairment (eGFR < 30 mL/min/1.73 m²)

Hepatic impairment
-
Hypersensitivity to Metformin or Glimepiride
-
Pregnancy and lactation (unless prescribed by a physician)
-
History of lactic acidosis
Warnings
-
Risk of lactic acidosis with metformin, especially in renal impairment.

Hypoglycemia risk with glimepiride, particularly with missed meals, alcohol intake, or strenuous exercise.
-
Monitor liver and kidney function regularly.
-
Use with caution in elderly, debilitated, or malnourished patients.
Dosage
-
As directed by the physician.

Usual starting dose: Once daily with meals, adjusted based on blood glucose response.

Do not crush or chew – swallow whole with water.
Interactions
-
Increased hypoglycemia risk: Sulfonylureas, insulin, beta-blockers, alcohol.

Reduced efficacy: Thiazide diuretics, corticosteroids, thyroid hormones, estrogens.

Metformin interaction: Contraindicated with iodinated contrast media (risk of lactic acidosis).
-
Monitor carefully with ACE inhibitors, NSAIDs, and certain antibiotics.
General Instructions
✅ Take with food to minimize GI effects.
✅ Follow prescribed diet and exercise plan.
✅ Regularly monitor blood glucose and HbA1c.
✅ Do not skip meals to avoid hypoglycemia.
✅ Avoid alcohol as it increases lactic acidosis & hypoglycemia risk.
✅ Inform doctor before surgery or contrast imaging.
