Summary

ETHKACIN-500MG contains Amikacin Sulphate, an aminoglycoside antibiotic used to treat severe bacterial infections caused by gram-negative organisms. It works by inhibiting bacterial protein synthesis, thus preventing bacterial growth and proliferation. It is typically used in hospital settings for infections of the respiratory tract, urinary tract, bones, joints, and skin.

Side Effects

  • Common: Nausea, vomiting, injection site reactions (pain, redness).
  • Serious: Hearing loss (ototoxicity), kidney damage (nephrotoxicity), balance issues, allergic reactions (rash, itching, swelling).
  • Rare: Respiratory paralysis in susceptible individuals.

Usage

  • Administered via intravenous (IV) or intramuscular (IM) injection under medical supervision.
  • Dosage varies based on body weight, type of infection, and renal function.
  • Ensure hydration to minimize nephrotoxicity risks.

Concerns

Monitoring: Regular monitoring of kidney function, hearing ability, and blood drug levels is required.

Resistance: Overuse or misuse can lead to antibiotic resistance

When not to use?

  • Known allergy to Amikacin or other aminoglycosides.
  • Patients with severe renal impairment or pre-existing hearing loss.
  • Myasthenia gravis: May worsen muscle weakness.
  • Pregnancy: Use only if absolutely necessary, as it may harm the fetus.

Warnings

  • Ototoxicity: Avoid in patients with auditory nerve damage.
  • Nephrotoxicity: Higher risk in elderly or dehydrated individuals; ensure proper hydration.
  • Neuromuscular blockade: Risk of respiratory paralysis in high doses or in combination with anesthetics or neuromuscular blockers.
  • Avoid prolonged use: To minimize toxicity risks.

Dosage

  • Adults: Typically, 15 mg/kg/day in divided doses every 8 or 12 hours. Adjustments needed for renal impairment.
  • Children and Neonates: Dosage based on weight and condition, under strict medical supervision.
  • Renal Impairment: Doses should be adjusted based on creatinine clearance.

Interactions

  • Medications: Enhanced nephrotoxicity with other nephrotoxic drugs (e.g., vancomycin, amphotericin B).
  • Anesthetics or neuromuscular blockers: Increased risk of respiratory depression.
  • Loop diuretics: Higher ototoxicity risk.
